Community-Led Health Transformation in Rural Oregon

We invested $14.6 million in the Oregon Healthiest State initiative between 2015 and 2020 to support the health and well-being goals of four remarkable rural communities - Klamath Falls, The Dalles, Grants Pass, and the Umpqua region. This report highlights the communities' efforts to improve the health and well-being of rural Oregonians.
