Palliative Care Access for Older Adults in Medicaid Funded Assisted Living

My project investigates palliative care access, social determinants of health, and aging in Medicaid-Funded Assisted Living Facilities (M-ALF). Preliminary evidence demonstrates M-ALF residents have palliative care need, palliative care stigma, mistrust for providers, and challenges secondary to social determinants of health. The proposal has two goals. 1) Map pathways to palliative care through M-ALF referral processes through qualitative interviews and organizational process mapping to identify opportunities to connect residents to services. 2) Develop and test the community health worker (CHW) Access to Care intervention which investigates the feasibility of providing tailored outreach, assessment, consultation and linkage with palliative care through a CHW.

My vision is to create equitable systems of palliative care support for older adults and family members as they transition from home to long term care. My professional goal is to implement interventions and leverage policy to better serve minoritized populations in Medicaid-Assisted Living Facility (M-ALF) that have minimal access to palliative care.
