Comunidades Unidas to Pilot Comprehensive Health Delivery Service Model in Utah

Innovative “Community to Clinic to Care” Program to reduce health disparities, introduce holistic care for Latinos in Utah and serve as a replicable model across the U.S.

SALT LAKE CITY, Utah  –  With support from the Cambia Health Foundation, Comunidades Unidas/Communities United (CU) is launching the Community to Clinic to Care Program, the first of its kind in Utah and one of the newest comprehensive health service delivery programs in the United States.  Cambia Health Foundation is investing in a two-year funding partnership with CU to pilot Community to Clinic to Care. The program utilizes expertly trained Community Health Workers (Promotoras de Salud) who are fully integrated in local clinics alongside medical providers, to ensure they receive patient-centered services that optimize their health and well-being.  Another project innovation is the use of telehealth technology to provide individuals with the opportunity to receive health care remotely when and where it is needed.

To develop and implement this model, CU is leveraging a strategic partnership with Utah Partners for Health’s Mid-Valley Health Clinic (MVHC).  Working together, integrated teams will directly connect patient care with consumer-centered prevention and education services in a way that is linguistically and culturally effective. MVHC is a Federally Qualified Health Center, located in Midvale, Utah, which provides primary health care services to 8,000 people annually, regardless of the ability to pay.  More than 70 percent of the people served at MVCH identify as Latino.  All staff are bilingual (English, Spanish, Portuguese and Tongan) and translation services are available as needed.
 
Cambia Health Foundation’s funding will help promote patient engagement and improve health outcomes for 3,000 low-income and underserved individuals; reach 10,000 people with relevant health information and healthy living messages; and graduate more than 50 community members from Community Health Worker training. While the Community to Clinic to Care Program will serve a predominately Latino population, it will reach a wide range of people in Utah.

Comunidades Unidas will officially launch the Community to Clinic to Care Program at an open house event on March 23, 2016 at 8:30 a.m. at 1750 W Research Way #102 in West Valley City. Comunidades Unidas emprende en nuevo modelo de salud en Utah enfocado en el paciente

Nuevo programa “Juntos hacia tu Salud” reducirá las desigualdades de salud, brindará cuidado de salud completo para los Latinos de Utah y será un modelo para el resto del país. 
 
SALT LAKE CITY, Utah – Con el apoyo de la Fundación Cambia, Comunidades Unidas lanzará el nuevo programa “Juntos hacia tu Salud,” el primero de este tipo en Utah y uno de los modelos de salud más innovadores de Estados Unidos. La Fundación Cambia ha invertido en un proyecto de dos años con Comunidades Unidas para crear este programa piloto. El programa utilizará a Promotoras de Salud que están integradas en el sistema de salud para trabajar en conjunto con proveedores de salud y asegurar que los servicios médicos sean adecuados y enfocados en el paciente y su bienestar. Utilizaremos tecnología Telehealth que permitirá brindar servicios de salud a larga distancia para personas que no puedan acudir directamente a un especialista.
 
Nuestra meta para este programa es auxiliar a las personas de bajos recursos, principalmente la población Latina, que padecen de diabetes o prediabetes, hipertensión o alto colesterol, a tomar un papel activo en el mejoramiento de su salud. Planeamos guiar a los individuos a reconocer las barreras que le impiden mejorar su salud y darles herramientas para superarlas con la ayuda de una Promotora de Salud y la colaboración con Mid-Valley Health Clinic. Esta combinación lograra darles a los clientes un servicio de calidad y donde se sentirán seguros.
 
En Comunidades Unidas buscamos informar, educar y empoderar al cliente acerca de su condición, sus derechos y responsabilidades y cómo navegar el sistema de salud de Utah. La promotora de salud también proveerá información sobre nutrición, ejercicio y solución de problemas que son específicas para las circunstancias de cada persona. Todo esto en un ambiente que es apropiado cultural y lingüísticamente.
