Important to the achievement of optimal health is an understanding of how social and physical environments influence health outcomes.
Social Determinants of Health
Social Drivers of Health (SDoH) are the conditions in the places where people live, learn, work and play that affect a wide range of health risks and outcomes. These factors include education, employment, food security, housing, access to health services, social isolation and transportation.

The health care industry's ability to address SDOH in partnership with individuals and their communities is key to improving population health. The Foundation is committed to expanding its knowledge and offers resources and information to help build awareness and an understanding of the implications of SDOH.
